Use your patient portal to request prescription refills, request appointments, view test results and immunization records.

If you don't have a Patient Portal account set up - please call the office and leave a voicemail with your full name, date of birth, and spell out your email address and we will activate your account!

You will need your statement code that is on your paper statement (new statements will be mailed out shortly).  If you do not have the code, you will need First and Last name, Date of Birth, and Phone number that is associated with your account.
