Use your patient portal to request prescription refills, request appointments, view test results and immunization records.
If you don't have a Patient Portal account set up - please call the office and leave a voicemail with your full name, date of birth, and spell out your email address and we will activate your account!
You will need your statement code that is on your paper statement (new statements will be mailed out shortly). If you do not have the code, you need First and Last name, Date of Birth, and Phone number that is associated with your account.

Dear Patients of Devon Family Practice,
We continue providing you medical care through a combination of televisits and in-person office visits. Televisits are done virtually with audio & visual communication via your smartphone, ipad/tablet, computer, or telephone. During the pandemic, televisits will be billed to your insurance company and copays/deductibles still apply in some cases. Each provider is also seeing a reduced schedule of in-person visits each day with enhanced safety protocols in place.
When you arrive for your in-person visit, please come into the office and check-in at the front desk. Please bring your current insurance card with you to your visit, and provide us with any updates to your address and phone number. There continues to be no formal check-out, so you will simply be billed for any applicable copays/deductible amounts.
Please contact our office with any questions or to schedule a televisit or in-person visit. We are experiencing an extremely high call volume at this time, and we ask for your patience. In addition to calling the office, you may send a message to your provider through your patient portal.
